Vertigo is a sensation of movement, typically perceived as a spinning sensation, which can be a symptom of various disorders affecting the vestibular system. It is vital to distinguish vertigo from other forms of dizziness, such as presyncope and disequilibrium, to ensure accurate diagnosis and treatment.
The clinical evaluation of vertigo includes a thorough patient history, physical examination, and, if necessary, diagnostic testing. The patient's description of their symptoms can often provide valuable clues to the underlying cause. Timing, triggers, and associated symptoms should be carefully assessed.
Diagnostic tests, such as electronystagmography (ENG), videonystagmography (VNG), and magnetic resonance imaging (MRI), can help identify the underlying cause. Central causes of vertigo, such as stroke or multiple sclerosis, typically require neuroimaging for diagnosis.
Management of vertigo depends on the underlying cause. Vestibular suppressants can provide symptomatic relief, but long-term use can hinder central compensation. Vestibular rehabilitation therapy (VRT) is often beneficial, particularly for peripheral causes of vertigo. In severe cases, surgical intervention may be required.
Vertigo is a complex clinical symptom requiring a comprehensive and systematic approach for effective management. Proper understanding of its pathophysiology, careful clinical evaluation, appropriate diagnostic testing, and targeted treatment strategies can significantly improve patient outcomes.
