Vertigo, a sensation of feeling off balance, is often associated with a condition affecting the inner ear or brain. It's a common complaint in clinical practice, yet it remains a diagnostic challenge due to its diverse etiologies and clinical presentations. This article provides a comprehensive clinical approach to vertigo, aiming to enhance the understanding, diagnosis, and management of this condition among healthcare professionals.
Vertigo is not a disease but a symptom of various conditions, most commonly vestibular disorders. It is characterized by a sensation of spinning or a feeling that the surroundings are spinning. This symptom is often accompanied by nausea, vomiting, nystagmus, or unsteadiness. Understanding the pathophysiology of vertigo is crucial for accurate diagnosis and effective treatment.
A systematic approach to the patient with vertigo begins with a detailed history and physical examination. The nature, duration, and frequency of vertigo, associated symptoms, and triggers should be thoroughly evaluated. Neurological and otological examinations, along with specific bedside tests, can help differentiate peripheral from central causes of vertigo.
Diagnostic modalities for vertigo include laboratory tests, imaging studies, and vestibular function tests. While laboratory tests are generally not specific, neuroimaging (CT or MRI) can be useful in ruling out central causes. Vestibular function tests, such as videonystagmography (VNG) and vestibular evoked myogenic potentials (VEMP), provide valuable information about the functional status of the vestibular system.
Management of vertigo depends on the underlying cause. In general, treatment may involve medications, vestibular rehabilitation, or surgical interventions. Providing patient education about the nature of the disorder and reassurance can also significantly improve outcomes.
Vertigo is a complex symptom with a multitude of potential causes. A systematic approach to history taking, clinical examination, and appropriate use of diagnostic modalities can help healthcare professionals accurately diagnose and effectively manage this challenging condition.
