Interstitial lung diseases (ILDs) represent a heterogeneous group of disorders characterized by inflammation and fibrosis of the pulmonary parenchyma. These disorders, often complex and challenging, require a comprehensive understanding for appropriate diagnosis and management.
ILDs can be idiopathic or associated with systemic diseases, environmental exposures, or drugs. The most common idiopathic form is idiopathic pulmonary fibrosis (IPF). Other forms include connective tissue disease-associated ILD, hypersensitivity pneumonitis, and drug-induced ILD. Classifying ILDs is crucial for prognosis and management.
Patients typically present with progressive dyspnea and non-productive cough. The physical examination may reveal inspiratory crackles and finger clubbing. High-resolution computed tomography (HRCT) is the cornerstone for diagnosis, often showing reticular opacities and honeycombing. Pulmonary function tests reveal a restrictive pattern with reduced diffusing capacity. Bronchoalveolar lavage and lung biopsy may be used in certain cases.
Management strategies depend on the underlying cause. For IPF, antifibrotic agents such as pirfenidone and nintedanib are recommended. In connective tissue disease-associated ILD, immunosuppressive therapy is often beneficial. Lung transplantation may be considered for select patients with advanced disease. All patients should receive supportive care, including pulmonary rehabilitation and oxygen therapy if needed.
The prognosis varies widely among ILDs. IPF has a poor prognosis with a median survival of 2-3 years from diagnosis. Meanwhile, other ILDs such as those associated with connective tissue diseases may have a more indolent course. Early diagnosis and appropriate management can improve the quality of life and potentially slow disease progression.
Understanding ILDs requires a comprehensive approach encompassing a thorough history, appropriate diagnostic tests, and a nuanced treatment plan. As healthcare professionals, staying informed about advances in the field is crucial to provide optimal care for patients living with these complex disorders.
