Vertigo, a common clinical symptom, presents as a sensation of spinning or feeling off-balance. It is often associated with a range of vestibular disorders. This article aims to provide a comprehensive clinical approach to diagnosing and managing vertigo.
Vertigo is not a disease but a symptom that can result from various conditions, most commonly peripheral vestibular disorders. It can also be a sign of central nervous system disorders. Differentiating between peripheral and central vertigo is crucial for accurate diagnosis and treatment.
An accurate history is the primary tool for diagnosing vertigo. The onset, duration, frequency, and associated symptoms can provide valuable clues. Physical examination, including a neurological examination and specific tests like the Dix-Hallpike maneuver, can help identify benign paroxysmal positional vertigo (BPPV).
Imaging studies like MRI or CT scans may be required to rule out central causes. Vestibular function tests, such as electronystagmography (ENG) or videonystagmography (VNG), can provide objective evidence of peripheral vestibular dysfunction.
Management of vertigo depends on the underlying cause. BPPV, the most common cause of vertigo, can often be treated with canalith repositioning maneuvers. Medications can alleviate symptoms in some cases, while vestibular rehabilitation therapy (VRT) can help patients cope with chronic vertigo.
Vertigo is a challenging symptom that requires a thorough and systematic approach to diagnosis and management. By understanding its various causes and utilizing appropriate diagnostic tools, healthcare professionals can effectively manage patients with vertigo and significantly improve their quality of life.
