However, the etiology can often be multifactorial, necessitating a thorough clinical evaluation. A detailed history and physical examination are the cornerstones of the diagnostic process. The use of validated questionnaires like the Leicester Cough Questionnaire can aid in quantifying the impact of the cough and monitoring treatment response.
Diagnostic testing should be guided by the clinical suspicion. Chest radiographs and spirometry are often the initial investigations. If these are inconclusive, further testing such as high-resolution computed tomography, bronchoscopy, or esophageal pH monitoring may be warranted. It is crucial to remember that negative tests do not exclude a diagnosis, and empirical treatment trials may be necessary.
Treatment of chronic cough should be etiology-specific. Pharmacological options include inhaled corticosteroids, proton pump inhibitors, or antihistamines, depending on the underlying cause. Non-pharmacological interventions such as cough suppression techniques and behavioral therapy can be beneficial. Refractory cases may require referral to specialist cough clinics.
In conclusion, chronic cough presents a diagnostic and therapeutic challenge for clinicians. A systematic and comprehensive approach to evaluation, guided by clinical suspicion, can aid in uncovering the underlying etiology. Tailored treatment strategies, encompassing both pharmacological and non-pharmacological interventions, can improve patient outcomes and quality of life.
