Chronic cough, persisting for over eight weeks, presents a significant diagnostic challenge in clinical practice. It can significantly impair quality of life, leading to social, physical, and psychological distress. Despite its prevalence and impact, a comprehensive approach to diagnosing and managing chronic cough remains elusive.
Initial evaluation should focus on identifying common causes: asthma, gastroesophageal reflux disease (GERD), and upper airway cough syndrome (UACS). A detailed medical history, physical examination, and basic investigations such as chest radiographs, spirometry, and laboratory tests can help identify these conditions. If the cause remains unclear, further investigations may include bronchoscopy, CT scans, or esophageal pH monitoring.
Treatment should be etiology-specific. For asthma-related cough, inhaled corticosteroids are the first-line treatment. GERD-related cough may respond to proton pump inhibitors, while UACS may require antihistamines or nasal corticosteroids. If no cause is identified, empirical treatment with cough suppressants, gabapentin, or amitriptyline may be considered. Non-pharmacological interventions, such as speech and language therapy and behavioral therapy, have also shown promise.
Refractory chronic cough, or cough unresponsive to standard management, requires a distinct approach. Recent research has highlighted the role of cough hypersensitivity syndrome (CHS) in these cases. CHS is characterized by heightened cough reflex sensitivity, often associated with laryngeal paresthesia and dysphonia. Neuromodulators, such as gabapentin and pregabalin, have shown efficacy in managing CHS, offering a new avenue for treatment.
Chronic cough is a complex clinical entity requiring a comprehensive and multidimensional approach. Recognizing common causes and using a systematic diagnostic process can help identify the underlying etiology. Tailored treatment strategies, including the use of neuromodulators for refractory cases, can improve patient outcomes and quality of life. Further research is needed to fully unravel the mystery of chronic cough and optimize its management.
