Parkinson's disease (PD) is a neurodegenerative disorder that predominantly affects dopaminergic neurons in the substantia nigra, a part of the brain that controls movement. This progressive illness is the second most common neurodegenerative disorder, following Alzheimer's disease.
The exact cause of PD remains elusive, but it's generally thought to arise from a combination of genetic and environmental factors. Pathologically, PD is characterized by the accumulation of alpha-synuclein protein in Lewy bodies and the loss of dopaminergic neurons in the substantia nigra.
PD manifests with both motor and non-motor symptoms. The cardinal motor symptoms include tremor, rigidity, bradykinesia, and postural instability. Non-motor symptoms such as autonomic dysfunction, cognitive impairment, and psychiatric symptoms can precede motor symptoms by several years.
There is no definitive test for PD. Diagnosis is primarily based on clinical criteria, including the presence of bradykinesia and at least one of the following: resting tremor, rigidity, or postural instability. Neuroimaging can be useful in differentiating PD from other parkinsonian syndromes.
PD treatment aims to manage symptoms and improve quality of life. Pharmacological therapy, primarily levodopa, remains the mainstay of treatment. Non-pharmacological interventions such as physical therapy and deep brain stimulation are also beneficial in managing PD symptoms.
Understanding the complex nature of Parkinson's disease is crucial for healthcare professionals. While we continue to unravel the mysteries of this debilitating disorder, ongoing research promises the development of better diagnostic tools and more effective treatments.
