Chronic cough, defined as a cough lasting more than eight weeks, poses a diagnostic challenge in clinical practice. Despite its prevalence, the underlying causes often remain elusive. This article aims to provide a comprehensive approach to diagnosing and managing chronic cough.
A thorough patient history and physical examination are paramount. Key factors include the duration and quality of the cough, associated symptoms, and exposure to potential triggers. Initial investigations should focus on common causes: upper airway cough syndrome (UACS), gastroesophageal reflux disease (GERD), and asthma. Chest radiography and spirometry are often beneficial.
Management should be etiology-specific. For UACS, first-line treatment includes nasal corticosteroids and antihistamines. GERD-related cough may respond to proton pump inhibitors. Inhaled corticosteroids are the cornerstone of asthma management. If the cough persists despite appropriate treatment, consider less common etiologies such as non-asthmatic eosinophilic bronchitis, chronic bronchitis, and bronchiectasis.
For refractory cases, a multidisciplinary approach is recommended. Collaboration between pulmonologists, gastroenterologists, allergists, and otolaryngologists can provide a holistic patient assessment and foster a more targeted therapeutic strategy. Speech-language pathologists can also play a crucial role in managing cough hypersensitivity syndrome.
Emerging diagnostic tools, such as impulse oscillometry and bronchial provocation tests, may help identify subtle abnormalities in lung function. Further research is needed to understand the pathophysiology of chronic cough better and develop more effective treatments.
Chronic cough is a complex clinical entity that requires a comprehensive, systematic approach for effective diagnosis and management. A multidisciplinary team can provide invaluable insights for challenging cases. As our understanding of chronic cough continues to evolve, so too will our diagnostic capabilities and therapeutic strategies.
