Pulmonary function tests (PFTs) are indispensable tools in respiratory medicine, providing objective data on lung function. However, their interpretation can be complex, demanding a thorough understanding of respiratory physiology. This article aims to elucidate the interpretation of PFTs for healthcare professionals.
Spirometry measures the volume of air that can be inhaled and exhaled, and the speed of the exhalation. The key parameters are Forced Vital Capacity (FVC), and Forced Expiratory Volume in one second (FEV1). The FEV1/FVC ratio is a crucial determinant of obstructive and restrictive lung diseases.
These tests evaluate the capacity of the lungs to transfer gases from inhaled air into the blood. Diffusing capacity of the lung for carbon monoxide (DLCO) is the most commonly used test. Reduced DLCO may indicate interstitial lung disease, emphysema, or pulmonary vascular disease.
These tests offer additional insights into lung mechanics. Total Lung Capacity (TLC), Residual Volume (RV), and Functional Residual Capacity (FRC) are key parameters. Increased RV and FRC suggest air trapping, commonly seen in obstructive diseases. Increased airway resistance may be seen in asthma and chronic bronchitis.
Exercise testing evaluates the integrated cardiopulmonary response to exercise. It can help identify exercise-induced bronchoconstriction, desaturation, or cardiovascular limitations to exercise.
PFTs provide a wealth of information about lung function. However, their interpretation requires a systematic approach, understanding of normal physiology, and correlation with clinical context. It is hoped that this guide aids healthcare professionals in interpreting PFTs, thereby facilitating accurate diagnosis and management of respiratory diseases.
