Tuberculosis (TB), caused by Mycobacterium tuberculosis, is a significant global health concern. Despite the availability of effective treatment, TB remains a leading cause of death from an infectious agent. The key to controlling the disease lies in prompt and accurate diagnosis. This article presents an overview of contemporary diagnostic techniques for TB.
Molecular diagnostics have revolutionized TB detection. Polymerase chain reaction (PCR) techniques, such as Xpert MTB/RIF, allow for rapid identification of TB and rifampicin resistance. Whole-genome sequencing (WGS) provides comprehensive genomic information, aiding in tracking disease transmission and predicting drug resistance. However, these techniques require sophisticated laboratory infrastructure, limiting their use in resource-poor settings.
Immunological methods like the tuberculin skin test (TST) and interferon-gamma release assays (IGRAs) are commonly used. IGRAs, such as QuantiFERON-TB Gold and T-SPOT.TB, offer higher specificity than TST, especially in BCG-vaccinated populations. However, these tests cannot differentiate between latent and active TB, necessitating further diagnostic evaluation.
Microscopy and culture methods remain the backbone of TB diagnosis in many settings. Ziehl-Neelsen staining and fluorescence microscopy are cost-effective methods for detecting acid-fast bacilli. Culture methods, although time-consuming, provide definitive diagnosis and allow for drug susceptibility testing.
Chest radiography and computed tomography (CT) scans are valuable tools in TB diagnosis. Radiographic findings, however, are not specific for TB, and abnormalities may persist even after successful treatment. The role of newer imaging techniques, such as positron emission tomography (PET), in TB diagnosis is currently being explored.
Though significant strides have been made in TB diagnostics, challenges persist. Molecular diagnostics offer rapid and accurate detection but are limited by cost and infrastructure requirements. Immunological tests and imaging techniques provide valuable information but lack specificity. The need for affordable, rapid, and accurate diagnostic tools is paramount in the fight against TB.
