Introduction

Interstitial lung diseases (ILDs) represent a diverse group of over 200 pulmonary conditions, characterized by inflammation and fibrosis of the lung interstitium. These disorders, while individually rare, collectively contribute to significant morbidity and mortality worldwide. This article aims to provide a comprehensive overview of ILDs to enhance clinicians' understanding and management of these complex conditions.

Etiology and Classification

ILDs can be idiopathic or associated with various systemic diseases, environmental exposures or drug toxicity. The most common ILD, Idiopathic Pulmonary Fibrosis (IPF), has an unknown etiology. Connective tissue diseases such as rheumatoid arthritis and systemic sclerosis are also frequently associated with ILDs. Occupational and environmental exposures, including asbestos and silica, can lead to specific forms of ILD like asbestosis and silicosis respectively.

Clinical Presentation and Diagnosis

Patients with ILD typically present with progressive dyspnea and non-productive cough. Physical examination may reveal inspiratory crackles and clubbing. High-resolution computed tomography (HRCT) is the imaging modality of choice for ILDs, providing detailed views of the lung parenchyma. Pulmonary function tests often show restrictive pattern with reduced diffusion capacity. Lung biopsy, though not always necessary, can help confirm diagnosis in uncertain cases.

Treatment and Prognosis

Treatment of ILDs is largely dependent on the underlying cause. For idiopathic forms, anti-fibrotic agents like pirfenidone and nintedanib have shown to slow disease progression. In ILDs associated with connective tissue diseases, immunosuppressive therapy is often beneficial. Lung transplantation may be considered for eligible patients with advanced disease. Prognosis varies widely among different ILDs, with median survival in IPF being approximately 3-5 years from diagnosis.

Understanding ILDs requires a comprehensive approach given their diverse etiologies, complex pathophysiology, variable clinical presentations, and differing treatment strategies. Continued research and advancements in diagnostic modalities and therapeutic options promise to improve the management and outcomes of these challenging diseases.
