Thyroid nodules, discrete lesions within the thyroid gland that are radiologically distinct from the surrounding thyroid parenchyma, are a common clinical finding. These nodules may be benign or malignant, and appropriate evaluation is essential for accurate diagnosis and treatment. Initial evaluation of a thyroid nodule includes a thorough history and physical examination. Risk factors for malignancy, such as family history of thyroid cancer, radiation exposure, and rapid nodule growth, should be identified. Physical examination should assess nodule size, consistency, mobility, and presence of cervical lymphadenopathy.
High-resolution ultrasound is the primary imaging modality for thyroid nodules. It can provide information about nodule size, shape, composition, echogenicity, and vascularity. Suspicious ultrasound features include microcalcifications, irregular margins, taller-than-wide shape, and increased intranodular vascularity.
Fine-needle aspiration biopsy (FNAB) is the most accurate and cost-effective method for evaluating thyroid nodules. It can distinguish between benign and malignant nodules, and guide further management. The Bethesda System for Reporting Thyroid Cytopathology provides a standardized approach to interpreting FNAB results.
Molecular testing can be used when FNAB results are indeterminate. It can identify specific gene mutations or rearrangements associated with thyroid cancer, helping to determine the need for surgery.
Evaluation of thyroid nodules is a complex process that involves a combination of clinical assessment, imaging studies, FNAB, and possibly molecular testing.
