Unexplained weight loss (UWL) represents a diagnostic challenge in clinical practice. Despite its association with underlying serious conditions, a structured approach to its evaluation is often lacking. This article aims to provide a comprehensive methodology to address this issue.
UWL, defined as a loss of more than 5% of body weight over 6-12 months without a known cause, is prevalent in 15-20% of older adults. It is associated with increased morbidity and mortality, often indicating underlying malignancy, gastrointestinal disorders, endocrine disorders, or psychiatric illnesses.
The initial evaluation should include a thorough history and physical examination. Clinicians should inquire about changes in diet, exercise, medication, and symptoms suggestive of systemic diseases. A comprehensive physical examination can reveal signs of chronic diseases or malignancies.
The diagnostic approach should be stepwise and cost-effective. Routine laboratory tests, including complete blood count, metabolic panel, thyroid function tests, and urinalysis, should be performed initially. If these tests are inconclusive, targeted imaging or endoscopic studies based on the patient's symptoms and signs should be considered.
Management should be tailored to the underlying cause. If a specific cause is identified, appropriate treatment should be initiated. In cases where no cause is found, the focus should be on symptom management and close monitoring for the emergence of potential underlying conditions.
UWL is a common yet challenging presentation in clinical practice. A systematic approach to its evaluation, starting from a thorough history and physical examination to a stepwise diagnostic approach, can help identify the underlying cause effectively and efficiently. Personalized management based on the identified cause can then be initiated, ensuring optimal patient care.
