Gastrointestinal bleeding (GIB) is a critical condition that requires prompt diagnosis and intervention. Its management can be challenging, given the broad range of potential etiologies and clinical presentations. This article aims to provide a comprehensive approach to the management of GIB.
Immediate assessment of hemodynamic stability and resuscitation if necessary is paramount. This includes fluid resuscitation, blood transfusion, and correction of coagulopathy. Rapid identification of the bleeding site via nasogastric lavage or rectal examination can be helpful.
Endoscopy remains the gold standard for diagnosing GIB. It allows for direct visualization and therapeutic intervention. Other modalities like computed tomography angiography can be used if endoscopy is not immediately available or contraindicated. Capsule endoscopy and angiography can be useful in certain cases of obscure GIB.
Therapeutic interventions for GIB include pharmacological therapy, endoscopic therapy, radiological intervention, and surgery. Proton pump inhibitors are the mainstay for upper GIB, while therapeutic endoscopy plays a significant role in both upper and lower GIB. Radiological interventions and surgery are reserved for refractory cases or when less invasive methods are unsuccessful.
Prevention of recurrence is an essential aspect of GIB management. This includes appropriate use of nonsteroidal anti-inflammatory drugs, antithrombotic therapy after careful risk-benefit analysis, and eradication of Helicobacter pylori when present.
Mastering the management of GIB involves a comprehensive approach that includes initial assessment and stabilization, appropriate use of diagnostic modalities, timely therapeutic interventions, and prevention of recurrence. Continuous education and staying updated with the latest evidence-based guidelines are key to improving patient outcomes in GIB.
