Acute Coronary Syndrome (ACS) represents a spectrum of conditions associated with acute myocardial ischemia. The management of ACS has dramatically evolved over the years with advancements in pharmacologic and interventional strategies. This article provides an overview of the current best practices in ACS management.
Over the past decade, substantial advancements have been made in the management of ACS, particularly in the realm of pharmacotherapy and percutaneous coronary intervention (PCI). The development of more potent antiplatelet agents like prasugrel and ticagrelor has significantly reduced the risk of recurrent ischemic events. The advent of bioresorbable vascular scaffolds and drug-eluting stents has improved the outcomes of PCI by reducing the risk of in-stent restenosis.
Early diagnosis and risk stratification using tools such as the GRACE score is crucial in guiding management. High-risk patients benefit from early invasive strategies including coronary angiography and revascularization. Dual antiplatelet therapy (DAPT) with aspirin and a P2Y12 inhibitor is the cornerstone of medical management. The choice of P2Y12 inhibitor should be individualized considering the patient's risk profile. Statin therapy should be initiated in all patients irrespective of their baseline LDL levels. Lifestyle modifications and cardiac rehabilitation form an integral part of secondary prevention.
Despite significant advancements, ACS remains a leading cause of morbidity and mortality worldwide. Future research should focus on refining risk stratification tools, developing novel therapeutic agents, and improving adherence to guideline-directed medical therapy. The potential of personalized medicine in optimizing ACS management is an exciting frontier to explore.
The management of ACS has come a long way, but the journey is far from over. As healthcare professionals, it is our responsibility to stay abreast with the latest advancements and best practices to provide optimal care to our patients. Continuous learning and adaptation are the keys to success in this ever-evolving field.
