The first step in diagnosing anemia is a complete blood count (CBC) and reticulocyte count. The CBC offers valuable clues about the type of anemia. For instance, low mean corpuscular volume (MCV) suggests microcytic anemia, often due to iron deficiency or thalassemia. High MCV points to macrocytic anemia, which may be caused by vitamin B12 or folate deficiency, myelodysplastic syndrome, or certain medications.
Additional tests depend on the initial findings. Iron studies, hemoglobin electrophoresis, or bone marrow examination may be warranted. For instance, a low serum ferritin level confirms iron deficiency anemia. Hemoglobin electrophoresis can diagnose thalassemia or sickle cell anemia. In cases of unexplained or refractory anemia, bone marrow examination may be necessary.
Interpreting test results should always consider the patient's clinical context. For example, anemia in a patient with rheumatoid arthritis may be due to chronic disease rather than iron deficiency. Similarly, a vegan may be more likely to have vitamin B12 deficiency anemia.
