Enteric fever, encompassing typhoid and paratyphoid fever, remains a significant global health issue. Despite advances in diagnostics and therapeutics, the disease continues to pose challenges due to its non-specific clinical presentation and the emergence of multi-drug resistant strains. This article aims to provide an updated approach to the diagnosis and management of enteric fever.
Early diagnosis of enteric fever is crucial to improving patient outcomes and preventing disease spread. Traditional culture methods, though considered gold standard, are time-consuming and lack sensitivity. Recent advancements in molecular diagnostics, such as polymerase chain reaction (PCR) assays, offer faster and more accurate detection of Salmonella Typhi and Paratyphi. However, their application in clinical practice is limited by cost and availability in resource-limited settings.
Antimicrobial therapy remains the mainstay of enteric fever management. The choice of antimicrobial should be guided by local resistance patterns. Fluoroquinolones, once the treatment of choice, are now less frequently used due to widespread resistance. Instead, third-generation cephalosporins and azithromycin have emerged as preferred options. The emergence of extensively drug-resistant (XDR) strains necessitates the use of newer agents like carbapenems and tigecycline.
Vaccination plays a crucial role in the control of enteric fever, especially in endemic regions. Two vaccines, Vi polysaccharide vaccine and Ty21a, are currently available and recommended for high-risk populations and travelers to endemic areas. Additionally, safe water and food practices, improved sanitation, and public health surveillance are pivotal in preventing the spread of enteric fever.
In conclusion, the management of enteric fever requires a multifaceted approach, encompassing rapid and accurate diagnostics, appropriate antimicrobial therapy guided by local resistance patterns, and preventive strategies including vaccination and improved sanitation. Keeping abreast with the latest developments in these areas is essential for clinicians involved in the care of patients with enteric fever.
