Unexplained weight loss, clinically known as involuntary weight loss (IWL), is a common yet perplexing condition encountered in primary care. It is often linked with numerous underlying diseases, making it a diagnostic challenge for healthcare professionals.
The causes of IWL can be broadly divided into three categories: malignancy, non-malignant gastrointestinal diseases, and psychiatric conditions. However, in about 16% of cases, the cause remains unknown, necessitating a comprehensive, patient-specific approach.
A thorough history taking and physical examination remain the cornerstones of the diagnostic approach. It is essential to scrutinize dietary habits, medication use, and psychosocial history. Laboratory tests, imaging studies, and endoscopy can be used to identify specific causes.
Management of IWL should be directed towards treating the underlying cause. In cases where no specific cause is identified, a symptom-directed approach can be adopted. Nutritional support, psychological therapy, and regular follow-ups are integral parts of management.
Unexplained weight loss remains a complex medical enigma. A comprehensive, patient-specific approach is required for its diagnosis and management. Further research is needed to improve our understanding and management of this challenging condition.
