Enteric fever, encompassing typhoid and paratyphoid fever, is a systemic infection primarily caused by Salmonella Typhi and Salmonella Paratyphi. Despite advancements in medical science, its diagnosis and management continue to pose challenges due to varying clinical manifestations and increasing antibiotic resistance.
Early diagnosis is crucial for effective management of enteric fever. Blood culture remains the gold standard for diagnosis, although it has limitations such as time consumption and low sensitivity. Other diagnostic modalities include bone marrow culture, stool culture, and serological tests like Widal test. However, these have their own set of limitations. Recent advancements include molecular techniques like PCR, which provide rapid and accurate results.
The increasing resistance of Salmonella species to first-line antimicrobials like chloramphenicol, ampicillin, and co-trimoxazole has necessitated the use of second-line drugs such as fluoroquinolones and cephalosporins. However, emerging resistance to these drugs has been reported. Therefore, susceptibility testing is essential for appropriate antimicrobial selection.
Vaccination plays a significant role in enteric fever prevention. Two vaccines, Vi polysaccharide vaccine and Ty21a vaccine, are currently available. Both vaccines provide a reasonable degree of protection and are recommended for travelers to endemic areas and high-risk populations.
In conclusion, enteric fever remains a significant global health issue. Early diagnosis and appropriate antimicrobial therapy are vital for successful management. Emerging diagnostic techniques and vigilant antimicrobial stewardship can help overcome the challenges posed by this disease. Vaccination remains an important preventive measure. Continuous research and development in these areas are crucial for the effective control and eventual eradication of enteric fever.
