Pulmonary hypertension (PH) is a complex and often misunderstood condition, with significant implications for patient morbidity and mortality. This article aims to provide a comprehensive review of the fundamentals of PH for healthcare professionals.
PH is defined as an increase in mean pulmonary arterial pressure ≥ 25 mmHg at rest, as assessed by right heart catheterization. The World Health Organization (WHO) classifies PH into five groups based on etiology, clinical presentation, hemodynamic characteristics, and treatment strategy.
Regardless of the cause, the pathophysiology of PH involves three main pathways: vasoconstriction, vascular remodeling, and thrombosis. These processes lead to increased pulmonary vascular resistance, right ventricular overload, and ultimately, right heart failure.
PH often presents with nonspecific symptoms such as dyspnea, fatigue, and syncope, making diagnosis challenging. A high index of suspicion is necessary, particularly in patients with risk factors such as connective tissue disease, congenital heart disease, and chronic lung disease.
The diagnostic evaluation of PH involves a combination of clinical assessment, echocardiography, and right heart catheterization. Additional tests may include pulmonary function tests, CT scan, and ventilation-perfusion scan to identify the underlying cause.
Treatment of PH is guided by the underlying etiology and the severity of disease. Strategies include general measures, targeted drug therapy, and in severe cases, lung transplantation. Early diagnosis and treatment can significantly improve patient outcomes.
In conclusion, PH is a complex condition requiring a multidimensional approach for diagnosis and management. Healthcare professionals must have a sound understanding of the fundamentals of PH to provide optimal care for these patients.
