Enteric fever, commonly known as typhoid fever, is a severe systemic infection caused by Salmonella Typhi and Paratyphi. Predominantly prevalent in regions with poor sanitation and hygiene, it remains a significant health concern. This article aims to delve into the effective management of enteric fever in clinical practice.
Enteric fever is primarily contracted through the ingestion of contaminated food or water. The clinical presentation can range from mild febrile illness to severe life-threatening complications. Symptoms include high fever, abdominal pain, constipation or diarrhoea, and rose spots on the chest.
Early diagnosis is crucial for effective management. Blood culture remains the gold standard for diagnosis, although it has a sensitivity of only 40-60%. Other diagnostic tests include stool and urine cultures, Widal test, and molecular methods like PCR for rapid detection.
Antibiotic therapy is the cornerstone of management. Empirical therapy with ceftriaxone or azithromycin is recommended until sensitivity results are available. Fluoroquinolones, once first-line treatment, are now reserved for resistant cases due to increasing resistance. Adjunctive therapy includes hydration and electrolyte management.
Vaccination is an effective preventive measure, especially for high-risk populations and travellers. Improvement in sanitation, safe food handling, and safe drinking water are also crucial in controlling enteric fever.
Enteric fever remains a significant public health issue, particularly in regions with inadequate sanitation. Early diagnosis, effective antibiotic therapy, and preventive measures are key to managing this condition. It is also important for healthcare professionals to stay updated on the evolving patterns of antibiotic resistance to ensure effective treatment strategies.
