Epilepsy, a chronic neurological disorder characterized by recurrent seizures, affects approximately 50 million people worldwide. Despite the availability of several antiepileptic drugs (AEDs), nearly one-third of patients continue to experience uncontrolled seizures. This article provides an overview of recent advancements in epilepsy management, aiming to enhance healthcare professionals' understanding and treatment approaches.
Over the last two decades, more than 15 new AEDs have been introduced, offering better safety profiles and fewer drug interactions. These include lacosamide, eslicarbazepine, and brivaracetam, which have shown efficacy in controlling focal seizures. Moreover, cannabidiol has recently been approved for use in Dravet and Lennox-Gastaut syndromes, marking a significant breakthrough.
Neuromodulation techniques such as Vagus Nerve Stimulation (VNS), Responsive Neurostimulation (RNS), and Deep Brain Stimulation (DBS) have emerged as promising therapeutic options for drug-resistant epilepsy. These techniques modulate neuronal activity, reducing seizure frequency and severity, and improving the quality of life.
Surgical intervention remains a viable option for patients with drug-resistant focal epilepsy. Advances in neuroimaging and surgical techniques have improved the precision and safety of epilepsy surgery. Minimally invasive procedures like Laser Interstitial Thermal Therapy (LITT) are now being used to ablate seizure foci, reducing recovery time and postoperative complications.
Genomic medicine has revolutionized the diagnosis and treatment of genetic epilepsies. Whole exome sequencing (WES) can identify pathogenic variants in known epilepsy genes, guiding personalized treatment strategies. Moreover, gene therapy is being explored as a potential cure for certain genetic forms of epilepsy.
These advancements in epilepsy management are transforming the treatment landscape, offering hope for better seizure control and improved quality of life for patients. As healthcare professionals, it is crucial to stay updated with these developments to provide optimal care for patients with epilepsy.
