Unexplained weight loss, or losing weight without trying, can be a cause of concern for both patients and physicians. It could be an indication of a serious underlying illness, necessitating a comprehensive medical approach for its assessment.
Initial evaluation should include a detailed history and physical examination. Key elements include dietary habits, physical activity, changes in taste or smell, psychiatric symptoms, and gastrointestinal symptoms. The history should also encompass inquiries about associated symptoms that may suggest a systemic disease.
Common causes of unexplained weight loss include malignancies, gastrointestinal diseases, endocrine disorders, and infections. In the elderly, depression, dementia, and social factors are also significant considerations. However, in a considerable proportion of patients, no clear cause may be identified even after extensive evaluation.
Basic laboratory tests include complete blood count, basic metabolic panel, liver function tests, thyroid function tests, and urinalysis. Further testing should be guided by the clinical suspicion based on the history and physical examination. Imaging studies, endoscopies, and biopsies may be required based on the initial findings.
Treatment should be directed at the underlying cause, if identified. For patients in whom no cause is identified, a symptom-based approach is recommended. Regular follow-up is essential to monitor the patient's weight, assess response to treatment, and identify any new symptoms or signs that may suggest a specific diagnosis.
Unexplained weight loss is a complex clinical problem that requires a comprehensive and systematic approach. While it can be a sign of a serious illness, a cause may not always be identified. Regular follow-up and a symptom-based approach are crucial in these cases to ensure optimal patient care.
