Identification of Hypertensive Emergencies

Patients with hypertensive emergencies often present with severe hypertension (systolic BP >180 mmHg or diastolic BP >120 mmHg). The key to identification is the recognition of end-organ damage, which may manifest as neurological, cardiovascular, renal, or retinal complications. Detailed clinical assessment and relevant investigations are essential to confirm the diagnosis.

Management Strategies

Initial management of hypertensive emergencies involves stabilization of the patient and reduction of BP. This should be done cautiously to avoid rapid reduction in BP, which may lead to end-organ hypoperfusion. Parenteral antihypertensive medications are often required for immediate BP control. The choice of medication depends on the specific organ system involved.

Latest Treatment Approaches

Recent advances in the treatment of hypertensive emergencies include the use of novel antihypertensive agents and targeted therapies. Sodium nitroprusside, once a mainstay of treatment, is being replaced by agents such as clevidipine and fenoldopam due to their better safety profiles. There is also growing interest in the use of renal denervation and baroreceptor activation therapy for resistant hypertension.
