Acute Coronary Syndrome (ACS) remains a leading cause of mortality and morbidity worldwide, necessitating the continuous evolution of its management strategies. This article aims to shed light on contemporary approaches to optimize patient outcomes in ACS management.
Pharmacotherapy forms the cornerstone of ACS management. Dual antiplatelet therapy (DAPT) comprising aspirin and a P2Y12 inhibitor remains the mainstay. Recent evidence suggests a shortened DAPT duration coupled with potent P2Y12 inhibitors like ticagrelor or prasugrel can reduce bleeding risks without compromising ischemic benefits. Additionally, novel anticoagulants like rivaroxaban have shown promising results in reducing adverse cardiac events in ACS patients.
Timely reperfusion, primarily through percutaneous coronary intervention (PCI), significantly improves survival in ACS. Advances in PCI techniques, including the use of drug-eluting stents and intravascular imaging, have improved its success rate. Furthermore, emphasis on 'door-to-balloon' time reduction and systems of care optimization have enhanced patient outcomes.
Non-invasive testing plays a pivotal role in risk stratification and guiding management in ACS. High-sensitivity cardiac troponin assays and novel biomarkers like copeptin and heart-type fatty acid-binding protein (H-FABP) have improved early diagnosis. Imaging modalities like coronary CT angiography and cardiac magnetic resonance imaging provide valuable prognostic information.
Secondary prevention measures including lifestyle modifications, cardiac rehabilitation, and optimal control of risk factors like hypertension and dyslipidemia are crucial in preventing recurrent events and improving long-term outcomes in ACS patients.
Contemporary ACS management involves a multi-faceted approach incorporating evidence-based pharmacotherapy, optimized reperfusion strategies, non-invasive testing, and robust secondary prevention measures. As our understanding of ACS evolves, it is imperative to continuously update our management strategies to improve patient outcomes.
