Peptic Ulcer Disease (PUD) represents a significant worldwide public health burden, with a high rate of morbidity and economic cost. However, in recent years, advancements in diagnostic techniques, pharmacotherapy, and endoscopic interventions have greatly improved the management of this condition.
Diagnosis of PUD has been revolutionized with the advent of non-invasive tests such as urea breath test and stool antigen test for Helicobacter pylori infection. These tests have high sensitivity and specificity, and are more patient-friendly than traditional invasive methods.
Advancements in pharmacotherapy have led to a significant reduction in ulcer recurrence and complications. Proton pump inhibitors (PPIs) remain the cornerstone of PUD management, providing rapid symptom relief and promoting ulcer healing. Novel PPIs with longer half-life and improved acid suppression are currently under investigation. Moreover, the introduction of quadruple therapy, including bismuth, has improved the eradication rates of H. pylori, the main causative agent of PUD.
Endoscopic interventions have transformed the management of PUD complications. Endoscopic hemostasis using clips, band ligation, and thermal devices has become the first-line treatment for bleeding peptic ulcers. In refractory cases, endoscopic ultrasound-guided angiotherapy is emerging as a promising technique.
The landscape of PUD management has significantly changed over the past few years, with advancements in diagnostic methods, pharmacological treatments, and endoscopic interventions. These developments have improved patient outcomes, reduced hospitalization rates, and decreased healthcare costs. However, further research is needed to refine these strategies and develop new therapeutic options to overcome the challenges associated with PUD management.
