Acute Coronary Syndrome (ACS) represents a spectrum of conditions associated with acute myocardial ischemia. Despite significant strides in understanding its pathophysiology, ACS remains a leading cause of morbidity and mortality worldwide. This article aims to outline recent advancements and best practices in ACS management.
Early diagnosis of ACS is crucial for effective management. High-sensitivity cardiac troponin assays have improved diagnostic accuracy, allowing for more rapid rule-in and rule-out of ACS. Risk stratification using tools such as the GRACE score aids in determining the most appropriate treatment strategy.
Pharmacotherapy remains a cornerstone in ACS management. Dual antiplatelet therapy (DAPT) with aspirin and a P2Y12 inhibitor is standard for most patients. Recent studies suggest that potent P2Y12 inhibitors, such as ticagrelor, may provide additional benefits. Novel anticoagulants, including low molecular weight heparins and direct oral anticoagulants, have shown promise in ACS treatment.
Early invasive strategy (coronary angiography with intent to perform revascularization) is recommended for high-risk patients. Percutaneous coronary intervention (PCI) remains the preferred method of revascularization. Advances in stent technology, such as bioabsorbable stents, have improved long-term outcomes.
Secondary prevention, including lifestyle modifications and management of risk factors, is essential in reducing recurrent events. Statins, ACE inhibitors, and beta-blockers have proven efficacy in secondary prevention. Cardiac rehabilitation programs have also been shown to improve outcomes.
Advancements in ACS management have significantly improved patient outcomes. Early diagnosis, risk stratification, optimal pharmacotherapy, early invasive strategies, and secondary prevention are key components in the management of ACS. Continued research and implementation of evidence-based practices will further enhance patient care in this critical area.
