Peptic Ulcer Disease (PUD) is a prevalent condition affecting millions worldwide. The past few years have witnessed significant strides in the understanding and management of this disease. This article provides a comprehensive review of these advancements and current best practices for PUD management.
Recent advancements in PUD management include the use of novel therapeutic agents and improved diagnostic techniques. Proton pump inhibitors (PPIs) and H2 receptor antagonists remain the mainstay of treatment, but recent studies have shown promising results with the use of potassium-competitive acid blockers (P-CABs) as an alternative to PPIs. Furthermore, the development of endoscopic techniques such as endoscopic mucosal resection and endoscopic submucosal dissection have enhanced the diagnostic accuracy and treatment of PUD.
Best practices in PUD management emphasize early diagnosis, appropriate use of pharmacotherapy, and lifestyle modifications. A combination of endoscopy and biopsy is the gold standard for diagnosing PUD. Pharmacotherapy should be individualized and typically involves PPIs or H2 receptor antagonists, along with eradication of Helicobacter pylori if present. Lifestyle modifications include dietary changes, stress management, and cessation of smoking and alcohol consumption.
Future directions in PUD management include the development of more effective and safer therapeutic agents, and implementation of precision medicine. The use of artificial intelligence in predicting disease course and response to treatment is an emerging field of interest. Additionally, the role of microbiome in PUD pathogenesis and treatment is currently under investigation.
In conclusion, the management of PUD has significantly evolved over the years, with advancements in therapeutic agents and diagnostic techniques, and emphasis on individualized treatment and lifestyle modifications. There is a need to keep abreast with these developments for optimal patient care. Future research should focus on developing safer and more effective treatment modalities, and exploring the role of precision medicine and microbiome in PUD management.
