Tuberculosis (TB), a leading cause of morbidity and mortality worldwide, poses significant diagnostic challenges. However, recent advances in diagnostic techniques are transforming the landscape of TB detection and management.
The advent of GeneXpert MTB/RIF, a rapid automated nucleic acid amplification test, has revolutionized TB diagnostics. It detects both TB and rifampicin resistance within hours, enabling prompt initiation of treatment. However, its reliance on sputum samples limits its utility in extrapulmonary TB and in children unable to produce sputum.
Line Probe Assays (LPAs) offer another molecular diagnostic approach, detecting specific genetic mutations associated with drug resistance. LPAs provide results within 48 hours, but require sophisticated laboratory infrastructure and trained personnel, limiting their use in resource-poor settings.
Computed Tomography (CT) scanning provides valuable diagnostic information in pulmonary TB, especially in cases with atypical radiographic presentations. CT scans can reveal characteristic findings such as cavitation and tree-in-bud appearance, enhancing diagnostic confidence.
Interferon-Gamma Release Assays (IGRAs) detect immune response to TB antigens, distinguishing between latent and active TB. IGRAs are more specific than traditional tuberculin skin tests, but cannot differentiate between TB infection and disease.
Whole Genome Sequencing (WGS) of Mycobacterium tuberculosis provides comprehensive genetic information, allowing detection of drug resistance, strain typing, and transmission tracking. However, its high cost and technical demands currently limit its widespread use.
In conclusion, the evolution of TB diagnostics is promising, with techniques such as GeneXpert and WGS offering rapid, accurate detection of TB and drug resistance. However, the global burden of TB necessitates further research and development to overcome limitations of current diagnostics, particularly in resource-limited settings.
