Most thyroid nodules are asymptomatic and discovered incidentally. However, some may present with compressive symptoms or thyroid dysfunction. Diagnosis is primarily through physical examination, thyroid function tests, and imaging studies, primarily ultrasonography.
Fine-needle aspiration biopsy (FNAB) is the gold standard for evaluating thyroid nodules. It helps in distinguishing benign from malignant nodules, guiding further management. The Bethesda System for Reporting Thyroid Cytopathology is universally used for interpreting FNAB results.
Management of thyroid nodules depends on the size, FNAB results, and patient's clinical context. Options range from surveillance to surgical intervention. Recent advances include minimally invasive techniques like radiofrequency ablation and laser ablation.
Regular follow-up is crucial in patients with thyroid nodules, especially those under active surveillance. This involves periodic clinical assessment, imaging, and repeat FNAB if necessary.
Thyroid nodule evaluation requires a systematic approach, incorporating clinical, biochemical, and radiological parameters. With advancements in diagnostic and therapeutic modalities, personalized management strategies can be implemented, ensuring optimal patient outcomes. Continued research is essential to further refine these strategies and address the challenges posed by this common clinical entity.
