Dyslipidemia, characterized by abnormal lipid levels, is a significant risk factor for cardiovascular disease (CVD). It is crucial for clinicians to understand its impact and implement effective treatment strategies in daily practice.
High levels of low-density lipoprotein cholesterol (LDL-C) and triglycerides, along with low levels of high-density lipoprotein cholesterol (HDL-C), contribute to the pathogenesis of atherosclerosis. This condition subsequently leads to CVD, including coronary artery disease, stroke, and peripheral artery disease. The global burden of dyslipidemia and its associated morbidity and mortality highlight the need for optimal management.
Statins remain the first-line therapy for dyslipidemia, reducing LDL-C levels and CVD risk. However, not all patients achieve their target LDL-C levels with statins alone. In such cases, ezetimibe or proprotein convertase subtilisin/kexin type 9 (PCSK9) inhibitors may be added. For patients with high triglyceride levels, fibrates or omega-3 fatty acids are recommended.
Alongside pharmacotherapy, lifestyle modification is fundamental in managing dyslipidemia. A diet low in saturated and trans fats, regular physical activity, weight management, and smoking cessation are crucial components of this approach.
Given the heterogeneity of dyslipidemia, a personalized approach to treatment is necessary. This involves considering the patient's overall CVD risk, comorbid conditions, potential drug-drug interactions, and patient preferences.
Managing dyslipidemia is a complex, multifaceted task requiring a comprehensive understanding of its impact, effective pharmacological interventions, lifestyle modifications, and the importance of individualized treatment strategies. With ongoing research and advancements, the future holds promise for further improving dyslipidemia management and reducing CVD risk.
