Abdominal pain, a common complaint in clinical practice, poses a diagnostic challenge due to its broad differential diagnosis. This article aims to provide a comprehensive approach to evaluating abdominal pain, focusing on key aspects of history taking, physical examination, and judicious use of investigations.
An accurate history is pivotal in narrowing down the differential diagnosis. Key components include the onset, location, character, radiation, associated symptoms, and alleviating or aggravating factors. A careful review of systems and past medical history can provide valuable clues to the underlying cause.
A systematic physical examination can reveal important signs. Inspection may reveal scars suggesting previous surgeries or distention indicating ascites. Palpation can identify tenderness, masses, or organomegaly. Auscultation helps assess bowel sounds, while percussion can detect free fluid or air. The examination should also include the genitourinary and rectal areas.
Investigations should be guided by the clinical suspicion. Basic investigations include complete blood count, liver and renal function tests, and urinalysis. Imaging studies such as ultrasound or computed tomography may be needed based on the clinical picture. More specialized tests like endoscopy or colonoscopy may be required for certain conditions.
Once the history, examination, and investigations are completed, a differential diagnosis can be formulated. This should be based on the likelihood of various conditions, taking into account the patient's age, sex, and risk factors. The differential diagnosis guides the management plan.
Evaluating abdominal pain requires a systematic approach, integrating information from the history, physical examination, and investigations. A comprehensive approach not only aids in reaching a correct diagnosis but also ensures efficient use of resources and optimal patient care.
