The mean corpuscular volume (MCV) can guide the differential diagnosis into microcytic, normocytic, or macrocytic anemia.
Microcytic anemia is often due to iron deficiency or thalassemia. Iron studies including serum iron, ferritin, total iron-binding capacity (TIBC), and transferrin saturation can differentiate these causes. Hemoglobin electrophoresis is crucial if thalassemia is suspected.
Normocytic anemia is typically characterized by a reticulocyte production index (RPI) that can differentiate between underproduction and hemolysis or blood loss. Further investigations like serum creatinine, thyroid-stimulating hormone (TSH), and vitamin B12 or folate levels may be necessary based on clinical suspicions.
Macrocytic anemia, often due to vitamin B12 or folate deficiency, requires serum measurements of these vitamins. If these are normal, consider myelodysplastic syndromes or drug effects. Bone marrow examination may be warranted in these cases.
Special populations, such as pregnant women or the elderly, may require different diagnostic approaches. Additionally, genetic testing and bone marrow biopsy may be necessary for certain cases.
Anemia's diagnostic approach requires a systematic and thoughtful evaluation. By understanding the pathophysiology and maintaining a high index of suspicion, clinicians can effectively diagnose and manage this common condition in the era of modern healthcare.
