Enteric fever, primarily caused by Salmonella Typhi and Paratyphi, remains a significant global health concern. Despite advancements in diagnostic tools and therapeutic strategies, the disease's management continues to pose challenges due to the emerging antibiotic resistance. This article outlines the updated approach to diagnosing and treating enteric fever.
Timely and accurate diagnosis of enteric fever is critical. Traditional culture methods, though considered the gold standard, are time-consuming and have low sensitivity. Therefore, serological tests like Widal and Typhidot have been widely used. However, their specificity and sensitivity have been questioned. Recent advancements include molecular techniques like PCR and next-generation sequencing, offering high sensitivity and specificity, though their application is limited in resource-poor settings.
Antibiotic therapy remains the mainstay of enteric fever treatment. However, the increasing prevalence of multi-drug resistant (MDR) and extensively drug-resistant (XDR) strains necessitates a change in the treatment approach. Fluoroquinolones, once considered first-line therapy, are now recommended as second-line due to resistance. Current guidelines suggest using azithromycin or ceftriaxone as first-line therapy for uncomplicated cases, while resistant cases may require newer agents like carbapenems.
Prevention strategies include improving sanitation, safe water supply, and vaccination. Ty21a and Vi polysaccharide vaccines have shown efficacy, though their protection is short-lived. The newly introduced conjugate vaccine (Typbar-TCV) offers longer protection and is recommended for endemic regions.
Enteric fever management requires an updated approach considering the changing epidemiological trends and emerging resistance. While newer diagnostic and therapeutic strategies offer hope, their application needs to be tailored to the local epidemiological and resource context. Furthermore, prevention through improved sanitation, safe water, and vaccination remains crucial in controlling the disease.
