Peptic Ulcer Disease (PUD) remains a prevalent gastrointestinal disorder with significant morbidity and mortality. As healthcare professionals, it is vital to adopt a comprehensive approach to its management to achieve optimal patient outcomes.
Understanding the pathophysiology of PUD is key to effective management. It involves an imbalance between the aggressive factors (H. pylori, NSAIDs, acid, pepsin) and defensive mechanisms (mucus, bicarbonate, blood flow, prostaglandins) of the gastric mucosa.
Accurate diagnosis and risk stratification are essential. This includes history taking, physical examination, laboratory tests, and endoscopy. High-risk features such as age over 60, presence of alarm symptoms, and NSAID use should prompt urgent endoscopy.
Treatment involves eradicating H. pylori, if present, with a regimen of proton pump inhibitors and antibiotics. For NSAID-induced ulcers, discontinuation of NSAIDs and long-term PPI therapy is recommended. In refractory cases, surgery may be necessary.
Prevention strategies include judicious use of NSAIDs, H. pylori screening and eradication, and patient education. Regular follow-up is crucial to assess treatment response, monitor for complications, and ensure adherence to therapy.
In conclusion, effective management of PUD requires a comprehensive approach that encompasses a thorough understanding of the pathophysiology, accurate diagnosis and risk stratification, appropriate treatment strategies, and vigilant prevention and follow-up. By adopting such an approach, healthcare professionals can significantly improve the outcomes and quality of life of patients with PUD.
