Peptic ulcer disease (PUD) continues to be a significant health concern worldwide, with advancements in diagnostic and therapeutic strategies significantly impacting disease management. This article presents an overview of the recent advancements in PUD management, providing healthcare professionals with a comprehensive guide to contemporary practices.
Endoscopy remains the gold standard for diagnosing PUD. However, non-invasive tests like urea breath tests, serology, and stool antigen tests have improved in accuracy, allowing for easier Helicobacter pylori detection. These advancements have facilitated quicker diagnosis and early initiation of treatment, reducing complications.
Pharmacotherapy for PUD has evolved significantly. Proton pump inhibitors (PPIs) remain the cornerstone of treatment, but the development of vonoprazan, a potassium-competitive acid blocker, has demonstrated superior efficacy in eradicating H. pylori. Additionally, the introduction of quadruple therapy, including a PPI, bismuth, and two antibiotics, has improved eradication rates, particularly in patients with antibiotic-resistant H. pylori.
Emerging evidence suggests a beneficial role for probiotics in PUD management. Probiotics can enhance the efficacy of H. pylori eradication therapy and reduce side effects, improving patient adherence to treatment. However, further research is needed to establish optimal probiotic strains and dosages.
For patients with complications such as bleeding ulcers, endoscopic therapies have seen significant advancements. Endoscopic hemostatic devices, including clips and bands, provide effective control of bleeding. Additionally, endoscopic submucosal dissection has emerged as a minimally invasive technique for treating gastric ulcers with low-grade dysplasia.
The landscape of PUD management is continually evolving, with advancements in diagnostic techniques, pharmacotherapy, probiotics, and endoscopic therapies improving patient outcomes. Ongoing research and development promise further enhancements in PUD management, emphasizing the need for healthcare professionals to stay abreast of these advancements to provide optimal patient care.
