Parkinson's disease (PD) is a chronic, progressive neurological disorder characterized by motor and non-motor symptoms. The disease is primarily associated with the degeneration of dopaminergic neurons in the substantia nigra, leading to various clinical manifestations.
PD is characterized by the loss of dopamine-producing cells in the substantia nigra, a region of the midbrain. This loss leads to an imbalance in the basal ganglia's neuronal activity, resulting in the cardinal motor symptoms of PD: bradykinesia, rigidity, and tremor. The exact cause of neuronal death remains unknown, but a combination of genetic and environmental factors is believed to contribute.
PD typically presents with motor symptoms such as resting tremor, bradykinesia, rigidity, and postural instability. However, non-motor symptoms like autonomic dysfunction, cognitive impairment, mood disorders, and sleep disturbances often precede motor symptoms and contribute significantly to patient disability.
Diagnosis of PD is primarily clinical, based on the presence of characteristic motor symptoms and the exclusion of other conditions. Currently, there are no definitive diagnostic tests for PD. Neuroimaging techniques, such as DaTSCAN, can support the diagnosis but are not routinely used due to their high cost and limited availability.
The mainstay of PD treatment is pharmacological therapy aimed at restoring dopamine levels in the brain. Levodopa, often combined with carbidopa, is the most effective treatment. Other drugs, such as dopamine agonists and MAO-B inhibitors, are also used. Non-pharmacological treatments, including physiotherapy and deep brain stimulation, may be beneficial in managing specific symptoms.
PD is a complex disorder with a multifaceted clinical presentation. Understanding its pathophysiology, clinical manifestations, and treatment options is essential for healthcare professionals to provide optimal patient care. Ongoing research aims to uncover the underlying mechanisms of PD and develop novel therapeutic strategies.
