Unexplained weight loss (UWL), defined as a loss of 5% of total body weight within a six-month period without a known cause, poses a diagnostic challenge in clinical practice. It can indicate underlying serious illnesses, and thus, necessitates a thorough evaluation.
The initial assessment for UWL should include a detailed history and physical examination. The history should focus on dietary intake, physical activity, psychiatric status, and associated symptoms. The physical examination should be comprehensive, including a thorough examination of the gastrointestinal system, lymph nodes, and thyroid gland.
A stepwise diagnostic approach is recommended for UWL. Initial laboratory tests should include a complete blood count, metabolic panel, thyroid function tests, and urinalysis. If these tests are inconclusive, further investigations such as imaging studies or endoscopies may be warranted. The choice of further tests should be guided by the patient's history and physical examination findings.
Imaging studies play a crucial role in the evaluation of UWL. Computed tomography (CT) of the abdomen and pelvis can identify malignancies, while positron emission tomography (PET) can detect metabolically active diseases. However, these should only be pursued if initial tests are inconclusive and the patient has suggestive symptoms.
Management of UWL depends on the underlying cause. For patients with malignancies or chronic infections, appropriate treatment should be initiated promptly. For patients with psychiatric disorders or malnutrition, a multidisciplinary approach involving psychiatrists and dietitians may be beneficial.
Evaluating unexplained weight loss requires a systematic approach, starting with a thorough history and physical examination, followed by a stepwise diagnostic process. This comprehensive approach can help clinicians identify serious underlying conditions and initiate appropriate management.
