A detailed history and physical examination form the cornerstone of peripheral neuropathy evaluation. Key features to assess include the pattern of symptoms, onset, progression, and associated systemic signs. A careful sensory examination can help distinguish peripheral neuropathy from other neurological disorders.
Once clinical suspicion is established, further investigations are warranted. Blood tests including complete blood count, serum electrolytes, liver and renal function tests, thyroid function tests, and Vitamin B12 levels can identify systemic conditions causing neuropathy. In cases of suspected hereditary neuropathies, genetic testing may be indicated.
Electrophysiological studies such as nerve conduction studies and electromyography are crucial in confirming the diagnosis, determining the extent and pattern of nerve involvement, and differentiating axonal from demyelinating neuropathies.
While not routinely performed, nerve or muscle biopsy can be considered in select cases where the diagnosis remains elusive or to confirm specific etiologies such as vasculitis or amyloidosis.
Imaging, including MRI of the spine or affected limbs, can be useful in cases of focal neuropathies or to rule out other causes of neurological symptoms such as spinal cord compression.
Peripheral neuropathy evaluation requires a comprehensive, systematic approach that includes a detailed clinical assessment, appropriate investigations, and specialized tests. Understanding the complexities involved in this process can assist healthcare professionals in making an accurate diagnosis, guiding treatment, and improving patient outcomes.
