Diagnosis of COPD

A post-bronchodilator FEV1/FVC < 0.70 confirms the presence of persistent airflow limitation, thus establishing the diagnosis of COPD. Additionally, the severity of airflow limitation, patient's symptoms, and risk of exacerbations should be assessed to guide treatment.

Pharmacological Treatment Strategies

Pharmacological therapy for COPD is primarily aimed at reducing symptoms, decreasing the frequency and severity of exacerbations, and improving health status and exercise tolerance. Bronchodilators are the cornerstone of symptomatic management. Inhaled corticosteroids (ICS) may be added to bronchodilator therapy in patients with a high risk of exacerbations.

Non-Pharmacological Treatment Strategies

For patients with advanced disease and those not responding to standard therapies, lung volume reduction surgery, endobronchial valves, coils, bullectomy, and lung transplantation may be considered. The future of COPD management lies in personalized medicine, with treatments tailored to the individual's genetic makeup, environment, and lifestyle.
