Enteric fever, encompassing both typhoid and paratyphoid fever, remains a significant global health concern. Despite advances in diagnostic techniques and therapeutic strategies, the disease continues to pose challenges due to increasing antibiotic resistance and diagnostic complexity. This article aims to provide an updated approach to the diagnosis and treatment of enteric fever.
Diagnosis of enteric fever is typically based on clinical presentation, followed by microbiological confirmation. Blood culture remains the gold standard, with sensitivity ranging from 40-80%. However, the advent of molecular diagnostic techniques, such as polymerase chain reaction (PCR), offers enhanced sensitivity and specificity. Rapid diagnostic tests (RDTs) targeting specific antigens have also emerged as promising tools, particularly in resource-limited settings.
Empirical antibiotic therapy should be initiated promptly in suspected cases of enteric fever. The choice of antibiotic should be guided by local susceptibility patterns. Fluoroquinolones, ceftriaxone, and azithromycin are commonly used. However, the rise of extensively drug-resistant (XDR) strains necessitates careful antibiotic stewardship and consideration of newer agents like typhoid conjugate vaccines (TCVs).
Preventive measures, including vaccination, play a crucial role in the management of enteric fever. TCVs, offering longer-lasting protection and suitability for children under two, provide a significant advancement in typhoid prevention. The World Health Organization recommends TCVs as a priority in areas with high typhoid burden or antibiotic resistance.
In conclusion, the management of enteric fever requires a comprehensive approach, integrating advanced diagnostic techniques, effective antibiotic therapy, and preventive strategies. The emergence of antibiotic resistance underscores the importance of ongoing research and innovation in this field. It is crucial for healthcare professionals to stay updated on these advancements to deliver optimal patient care.
