Introduction

Peptic ulcer disease (PUD) remains a prevalent health concern worldwide, affecting approximately 4.6 million people annually. With the advent of new technologies and therapeutic strategies, the management of PUD has seen considerable advancements. This article provides a comprehensive overview of the latest developments in PUD management, aimed at enhancing healthcare professionals' understanding and treatment approach.

Diagnosis

While endoscopy remains the gold standard for diagnosing PUD, non-invasive diagnostic techniques have seen significant improvements. The Helicobacter pylori stool antigen test and the urea breath test, for instance, offer high sensitivity and specificity. Furthermore, serologic tests have been refined to detect H. pylori antibodies, providing an economical and less invasive diagnostic alternative.

Pharmacological Management

Pharmacological therapy for PUD has evolved significantly. The traditional regimen of proton pump inhibitors (PPIs) and antibiotics for H. pylori eradication remains effective, but the rise in antibiotic resistance necessitates alternative strategies. Quadruple therapy, including a PPI, bismuth, and two antibiotics, has emerged as a potent alternative. Moreover, Vonoprazan, a potassium-competitive acid blocker, has shown superior efficacy to PPIs in certain populations.

Endoscopic Techniques

Endoscopic treatment modalities have also advanced, providing alternative solutions for refractory or complicated ulcers. Endoscopic submucosal dissection (ESD) and endoscopic mucosal resection (EMR) are now widely used for gastric ulcer treatment, reducing the need for surgical intervention. Additionally, endoscopic hemostasis techniques, such as endoscopic clipping and argon plasma coagulation, offer effective management for ulcer bleeding.

Prevention Strategies

Preventive measures have been enhanced by the development of effective H. pylori vaccines, currently in clinical trials. Additionally, the judicious use of non-steroidal anti-inflammatory drugs (NSAIDs) and the co-prescription of PPIs in high-risk patients have been emphasized in recent guidelines to reduce ulcer incidence.

Conclusion

Managing peptic ulcer disease has significantly evolved with advancements in diagnostic tools, pharmacological therapies, endoscopic techniques, and preventive strategies. As healthcare professionals, staying abreast of these developments is essential to provide optimal patient care. Continued research and innovation are expected to further improve PUD management, promising a future with better patient outcomes and decreased disease burden.
