Peptic Ulcer Disease (PUD) remains a significant health concern worldwide, with an increasing prevalence due to lifestyle changes and Helicobacter pylori infection. The management of PUD has transformed significantly over the years, with advancements in diagnostic techniques, pharmacotherapy, and endoscopic interventions.
Advancements in diagnostic techniques have improved the detection and evaluation of PUD. The introduction of high-resolution endoscopy and narrow-band imaging has enhanced visualisation of the mucosal surface, enabling the detection of subtle lesions. Furthermore, the use of H. pylori stool antigen test and urea breath test has increased the accuracy of H. pylori detection.
The cornerstone of PUD treatment remains the eradication of H. pylori. The advent of quadruple therapy, including a proton pump inhibitor (PPI), bismuth, and two antibiotics, has improved eradication rates. Additionally, the development of vonoprazan, a potassium-competitive acid blocker, has shown superior acid-suppressing effects compared to traditional PPIs, potentially enhancing ulcer healing.
Endoscopic interventions have revolutionized the management of PUD complications, such as bleeding ulcers. Techniques such as endoscopic clipping, band ligation, and argon plasma coagulation have demonstrated high efficacy in achieving hemostasis. Moreover, endoscopic submucosal dissection offers a minimally invasive approach to manage gastric neoplasms associated with PUD.
Advancements in PUD management have significantly improved patient outcomes, with enhanced diagnostic accuracy, effective pharmacotherapy, and innovative endoscopic interventions. However, the increasing prevalence of antibiotic-resistant H. pylori strains necessitates continued research and innovation in this field. As healthcare professionals, staying abreast with these advancements is integral to delivering optimal patient care.
