Cardiovascular diseases (CVD) remain a leading cause of morbidity and mortality among patients with diabetes. This article provides a comprehensive guide on how to assess and mitigate cardiovascular risk in diabetic patients.
Assessment of cardiovascular risk in diabetic patients should be multifactorial. It should include traditional risk factors like hypertension, hyperlipidemia, and smoking. Use of risk calculators like the Framingham Risk Score can aid in quantifying cardiovascular risk.
Hyperglycemia is a potent cardiovascular risk factor. Intensive glycemic control can reduce microvascular complications but its effect on macrovascular complications is less clear. Nevertheless, individualized A1C targets should be set for diabetic patients considering their age, comorbidities, and hypoglycemia risk.
Both hypertension and dyslipidemia significantly contribute to cardiovascular risk. Blood pressure should be controlled to less than 140/90 mmHg in most diabetic patients. Statins should be initiated for diabetic patients over 40 years, or those with additional risk factors regardless of their LDL levels.
Low-dose aspirin for primary prevention of CVD in diabetes is controversial. It may be considered in patients with increased cardiovascular risk and no contraindications. However, the benefits should outweigh the risk of gastrointestinal bleeding.
Healthy lifestyle modifications including regular exercise, a balanced diet, smoking cessation, and weight control are essential components of cardiovascular risk mitigation in diabetic patients.
In conclusion, cardiovascular risk assessment and mitigation in diabetic patients should be multifactorial and individualized. It should encompass not just pharmacological interventions but also lifestyle modifications.
