Cirrhosis involves the replacement of healthy liver tissue with fibrous scar tissue, disrupting the organ's ability to perform vital functions such as detoxification, protein synthesis, and production of clotting factors. This can lead to a variety of complications.
Ascites, variceal bleeding, hepatic encephalopathy, and hepatorenal syndrome are among the most common complications. These can significantly impact patients' quality of life and survival rates.
Ascites management typically involves sodium restriction, diuretics, and therapeutic paracentesis. Refractory cases may require transjugular intrahepatic portosystemic shunt (TIPS) or liver transplantation.
Prevention of variceal bleeding involves non-selective beta-blockers or endoscopic band ligation. Acute bleeding requires endoscopic therapy, vasoactive drugs, and antibiotics.
Hepatic encephalopathy management involves lactulose and rifaximin to reduce ammonia levels. Identifying and treating precipitating factors is also crucial.
Hepatorenal syndrome requires albumin and vasoconstrictors, with TIPS or liver transplantation as potential options for refractory cases.
