Immediate management involves reducing blood pressure cautiously. The aim is not to normalize blood pressure rapidly, but to reduce it by no more than 25% within the first hour. Intravenous antihypertensive agents, such as labetalol or sodium nitroprusside, are often used. Continuous monitoring of blood pressure is essential during this phase.
Following stabilization, long-term management aims to maintain blood pressure within the normal range and prevent further end-organ damage. This typically involves a combination of lifestyle modifications and pharmacological therapy. Regular follow-up and monitoring are key to ensure optimal blood pressure control.
Untreated or inadequately treated hypertensive emergencies can lead to catastrophic complications including stroke, myocardial infarction, and renal failure. With appropriate management, the prognosis can be significantly improved. However, long-term control of hypertension remains a challenge requiring a multidisciplinary approach.
