Unexplained weight loss (UWL), defined as a loss of more than 5% of body weight within six months without an identifiable cause, poses a significant diagnostic challenge for clinicians. This condition can be a harbinger of serious underlying diseases, making timely and accurate diagnosis crucial.
UWL can be attributed to a multitude of causes, including malignancies, gastrointestinal disorders, endocrine diseases, and psychiatric illnesses. In the elderly, UWL is common and often linked to malignancies and nonmalignant gastrointestinal diseases. However, in up to 25% of cases, the cause remains elusive despite extensive evaluation.
A comprehensive history and physical examination serve as the cornerstone for diagnosing UWL. Clinicians should focus on dietary habits, medication use, family history, and associated symptoms. Laboratory tests, imaging studies, and endoscopic procedures may be warranted based on the clinical suspicion.
While there is no single diagnostic test for UWL, a combination of tests can be useful. Complete blood count, metabolic panel, thyroid function tests, and cancer screenings are often employed. Advanced imaging techniques and endoscopic procedures can be beneficial in detecting occult malignancies and gastrointestinal disorders.
Management of UWL hinges on treating the underlying cause. In cases where no cause is found, symptom management and close follow-up are essential. Regular monitoring can help detect any new symptoms or signs that may point to a specific diagnosis.
UWL is a complex clinical scenario that requires a thorough and systematic approach. While it can be a symptom of various serious illnesses, in a significant proportion of cases, no cause is found. A meticulous history, physical examination, and judicious use of diagnostic tests are key to identifying the underlying cause and guiding management.
