Gastrointestinal bleeding (GIB) is a common medical emergency, often presenting diagnostic challenges due to its diverse etiologies. A systematic approach is necessary to accurately diagnose and manage GIB. This article provides a comprehensive guide to the diagnostic approach for GIB.
GIB can originate from anywhere within the gastrointestinal tract. Upper GIB typically presents with hematemesis or melena, while lower GIB manifests as hematochezia. The etiology often determines the clinical presentation, with peptic ulcer disease, esophageal varices, and diverticulosis being common causes.
Initial assessment should focus on hemodynamic stability, followed by a thorough history and physical examination. Fluid resuscitation and blood transfusion may be necessary in unstable patients. Risk stratification tools like the Glasgow-Blatchford score can guide management decisions.
Endoscopy is the primary diagnostic modality for GIB. Upper endoscopy is indicated for suspected upper GIB, while colonoscopy is preferred for lower GIB. Computed tomography angiography can be used when endoscopy is non-diagnostic or contraindicated. Capsule endoscopy and angiography serve as additional tools for obscure GIB.
Management strategies depend on the etiology and severity of GIB. Endoscopic therapy is first-line for most causes of GIB. Pharmacologic therapy, including proton pump inhibitors and vasoactive drugs, is often adjunctive. Surgery or interventional radiology may be necessary for refractory or recurrent bleeding.
Gastrointestinal bleeding is a complex clinical condition requiring a systematic approach for effective diagnosis and management. Understanding the etiology, initial assessment, diagnostic modalities, and management strategies is crucial for optimal patient outcomes. Continuous advances in diagnostic and therapeutic modalities promise to further improve the care of patients with GIB.
