Enteric fever, primarily caused by Salmonella Typhi and Paratyphi, continues to pose significant health challenges globally. This article provides an updated approach to the effective management of enteric fever in clinical practice, focusing on diagnosis, antimicrobial therapy, and prevention strategies.
Early and accurate diagnosis is crucial for effective management of enteric fever. Blood cultures remain the gold standard for diagnosis, with sensitivity ranging from 40-80%. However, due to limitations such as turnaround time and requirement of laboratory facilities, rapid diagnostic tests (RDTs) like Typhidot and Tubex TF have been developed. These tests, based on detection of specific antibodies, provide results within 2-4 hours and are particularly useful in resource-limited settings.
Due to increasing antimicrobial resistance, the choice of empirical therapy has evolved over time. Fluoroquinolones, once first-line therapy, are now recommended only when susceptibility is confirmed. Current guidelines recommend azithromycin as first-line therapy for uncomplicated cases, with third-generation cephalosporins or carbapenems used for severe or resistant cases. Antimicrobial stewardship programs are essential to monitor resistance patterns and guide therapy.
Prevention of enteric fever primarily involves improving sanitation and access to clean water. Vaccination also plays a key role, with two vaccines currently available: the Vi polysaccharide vaccine and the Ty21a live-attenuated vaccine. The World Health Organization recommends vaccination in endemic areas and for high-risk populations.
In conclusion, the effective management of enteric fever involves a multifaceted approach, including accurate diagnosis, appropriate antimicrobial therapy, and robust prevention strategies. As the landscape of enteric fever continues to evolve with increasing antimicrobial resistance, it is vital for healthcare professionals to stay updated with the latest guidelines to ensure optimal patient care.
