Gestational diabetes mellitus (GDM) is a significant health concern affecting approximately 7% of all pregnancies. It not only impacts maternal health but also has long-term effects on the offspring. This article presents a comprehensive guide to the management of GDM to optimize outcomes for both mother and child.
Early detection is crucial in managing GDM. Universal screening, using oral glucose tolerance test, is recommended between 24 and 28 weeks of gestation. High-risk individuals should be screened at the first prenatal visit.
Medical nutrition therapy is the cornerstone of GDM management. A registered dietitian should provide individualized meal planning. The goal is to provide adequate nutrition for pregnancy while maintaining euglycemia and preventing excessive weight gain.
Regular physical activity complements medical nutrition therapy in controlling blood glucose levels. Moderate-intensity aerobic activity for at least 150 minutes per week is recommended unless contraindicated.
If lifestyle modifications fail to achieve glycemic control, pharmacotherapy is indicated. Insulin is the first-line therapy, but oral hypoglycemic agents like metformin and glyburide are also options.
Postpartum care is essential as GDM increases the risk of type 2 diabetes. Women should be screened for persistent diabetes at 4 to 12 weeks postpartum and every 3 years thereafter. Lifestyle modifications should be continued postpartum.
Optimal management of GDM requires a multidisciplinary approach involving early detection, medical nutrition therapy, physical activity, pharmacotherapy, and postpartum care. Through these measures, we can improve outcomes for both mother and child, reducing the risk of future metabolic disease.
